Parent E-PACK

This year, instead of distributing a hard copy Family Packet, we have compiled and simplified all forms into an easy electronic format! Please carefully complete each of the items below and provide your electronic signature after the last item.  On the Emergency Form, you will be asked to enter contact information for your child's health providers.  For the Parent/Student Handbook section, you are required to read through the handbook and review our policies with your child(ren). Upon completion, all families are required to print, sign and submit the last two pages to Hillel’s office, acknowledging that you have read all the information and agree to follow all of Hillel Community Day School’s policies. All items must be completed before the first day of school on Tuesday, September 4, 2012.
